Your journey with us

From referral to discharge

No matter which pathway you are on, every patient at Shama Health follows the same journey. Here is the complete picture — from your GP sending a referral through to being discharged back to their care.

Intake — Step 1
1

GP referral received

Your GP sends a referral letter to admin@shamahealth.com.au. The referral should include your reason for referral, relevant history, current medications, and your contact details. Our admin team acknowledges receipt within 1 business day.

Referral Triage — Step 2

Not accepted

  • The referral is outside our clinical scope
  • The patient requires a higher level of care
  • We are at capacity for this pathway

Your GP will be notified and alternative options will be suggested.

Accepted

Your referral is matched to the most appropriate clinician based on your needs and their areas of expertise.

Intake pathway — Steps 3 & 4
3

Intake forms completed

Once accepted, our team emails you a set of intake questionnaires. Complete them at your own pace and return by email — there is no deadline, but earlier return allows us to book your appointment sooner.

4

We call to book your appointment

Once your forms are received, our admin team calls you to schedule your first appointment at a time that suits you.

Clinical pathway — Steps 5 to 7
5

Initial nurse practitioner assessment

Your first appointment is with our nurse practitioner. This session gathers a full clinical history and prepares a summary for the consulting psychiatrist.

6

Psychiatrist consultation

You meet with your allocated psychiatrist for a comprehensive assessment. A diagnosis and management plan is formulated during or after this appointment.

7
Ongoing care and review

Your care plan is tailored to your needs. Review appointments are scheduled regularly to monitor progress, adjust medications, and provide ongoing support.

8

Discharge back to GP

When your treatment goals have been met, your psychiatrist prepares a discharge summary and transitions your ongoing care back to your GP. You remain welcome to re-refer in the future.
